While we have and will always respect your privacy, a new federal law now requires us to maintain the privacy of hearing health information (including examination, treatment, and billing records) about  you and to provide you with this notice of our legal duties and pricacy practices with respect to such information. We mus abide by the terma of this notice while it is in effect. However we reserve the right to change the terms of our privacy notice.. If we change the terms of the notice we will notify you during your next visit or by mail.

To maintain compliance with the Health Insurance and portability and accountability act of 1996 (HIPPA) , Oakdale hearing aid CO. has established the following guidelinnes to protect your personal health information. This policy has been implemented to  protect health information  received or created in the course of providing service to you. Any information that relates to your health or the care or service you have received, or payment fort that care or service that could be used to identiify you is considered protectec health information (PHI).We will not disclose your PHI for any reason without express written permission from you except as described below.

• To provide health care services or products. For example , to manufactures of hearing aids and ear molds  to make your hearing aids.
• To bill for producs or services to a third party such as an insurance company or other payor.
• To other health care providers if it is necessary to refer you to them or they are otherwise involved in your care when such information is required for them to treat you, receive payment for services, or conduct certain health care operations such as quality assesment and improvement activities, reviewing the quality and competence of health care professionals or for health care fraud and abuse detection or compliance.
• We may discolse information to one of your family members, other relative, close personal friend or any other person identified by you when you are present for , or otherwise available prioe to the disclosures, please notify us.
• To federal state or local law enforcement or oversight agencies as required to do so by law.

Your Rights

• You have the right to receive confidential communication regarding your health information which we normally provide to you in person at the time you receive hearing care services from us. We may also mail you information regarding your or about the status of your account. We will do our best to accomondate any reasonable request if you would like to receive information about your health or services that we provide by analternate means of communication or at an alternate location. To help us respond to your needs, please make any requests in writting.
• You have the right to review and obtain a copy of your PHI. You may be charged a reasonable fee for copying services. You will not be provided with information that would affect our ability to do business, for example, manufactures invoices or other correspondence that does not pertain to your health information, but might contain sensitive information such as  pricing and cost of services.
• You have the right to request additional restrictions on our use and disclosure pf your PHI. All request for such restrictions must be  made in writting and while we consider all restrictions carefully we are not required to agree to a resquested restriction.
• You have the right to receive an accounting of the disclosures we have made of your PHI for the last six years before the date of your request. This accounting will include all disclosures except thoes required to carry out treatment, payment, and healthcare operations,  you made pursuant to an authorized individual involved in your case, required fornational security or intelligence purposes, to correctional institutions or law enforcement officers

  If you desire further information about your privacy rights, are concerned that we violated your privacyrights or disagree with a decision that we made about providing you acess to your health information, please contact us. You may also file a written complaint with the Director,office of Civial rights, U.S. Derpartment of Health and Human Services, 200 Independence Ave. S.W. Washington, D.C. 20201. We respect your right to file a complaint and will not take any action against you if you file a complaint.
